Oral & Maxillofacial Surgery Associates sees patients by appointment only.  We are a referral-based office which means you must be referred by a dentist, dental specialist, or in some cases a physician in order to make an appointment.

We are open Monday – Thursday from 8:00 am until 5:00 pm and Friday from 8:00 am until 3:00 pm.

Because we value your time and want to make your visit with us as smooth as possible, we ask that you prepare a few items before your appointment:

• Fill out your patient registration and health history in advance.  For your convenience, we have Online Patient Registration Forms.  Please register online after you have made an appointment.
• Bring dental and medical insurance information with you. Some oral and maxillofacial surgical procedures are covered by both types of insurance. This will ensure that you receive the maximum benefit from your coverage, and allow timely and accurate claims submission to your carriers.
• Bring any referral slip and/or X-Ray(s) from your dentist or physician to help us optimize your care. The referring office can email, fax, or mail your referral slip directly to our office.  If you do not have a current x-ray, we can take one here at our office.     

Please note, patients under the age of 18 must be accompanied by a parent or guardian. For the courtesy of our surgical patients who may be fasting, we kindly ask that no food or drink be brought to the office.

New Patients & Consultations: Please arrive 20 minutes prior to scheduled appointment time.
Surgeries: Please arrive 15 minutes prior to your scheduled appointment time.
Follow-ups: Please arrive 10 minutes prior to your scheduled appointment time.

Missed Appointment and Late Cancellation/Reschedule Policy: 

A missed appointment is costly to a physician practice, patients, and the industry of healthcare. Missing an appointment may pose a serious health risk to you and is unfair to other patients who have been waiting to get an earlier appointment. Our policy is to charge for missed appointments. We ask that if you must cancel or reschedule your appointment to notify us no later than three business days in advance to avoid missed appointment fees. Missed appointment charges are your responsibility and will be billed directly to you. Appointments will not be rescheduled until payment is received for missed appointments. Please know that we may elect to terminate your relationship with the practice for missed appointments.

Consultation Visit Missed Appointment Fee Schedule:

• First Missed Appointment: Warning – No Charge
• Second Missed Appointment: $100.00
• Third Missed Appointment: $100 + Possible termination from the practice

Procedure Missed Appointment Fee Schedule

• First Missed Procedure: $200.00
• Second Missed Procedure: $200 + Possible termination from the practice

A pre-operative consultation and physical examination may be required depending on the type of treatment you need. For your safety, we may also require a consult if you have a complex medical history. At your initial consultation, the doctor will evaluate you and recommend a treatment plan. Typically, surgery does not occur at this visit.

There are fees for initial consultations. If you have insurance, a deposit/copay may be due at the time of your visit but we will gladly bill your insurance for you.
