The Joint Commission will conduct a re-accreditation survey of Oral Surgery Associates on Monday, April 25th, 2022
The purpose of the survey is to evaluate the practice’s compliance with nationally established Joint Commission standards. The survey results will be used to determine whether, and the conditions under which, accreditation should be awarded to the practice.
Anyone believing that they have pertinent and valid information about quality of care issues and the safety of the environment in which care is provided may request an interview.
If anyone has concerns about patient care and safety in the organization, that the organization has not addressed, you are encouraged to contact the organization’s management. If you feel the concerns were not resolved through the organization, you are encouraged to contact the Joint Commission via the website www.jointcommission.org and using the “Report a Patient Safety Event” link in the Action Center, by fax to 630-792-5636 or by mail to Office of Quality and Patient Safety, The Joint Commission, One Renaissance Boulevard, Oakbrook Terrace, IL 60181.
The Joint Commission will acknowledge such requests in writing or by telephone.
This notice is posted in accordance with the Joint Commission’s requirements and may not be removed before the survey is completed.
